Job summary

Qualifications

• RN licensure and active licensure required.
• Experience in discharge planning or care coordination preferred.
• Ability to navigate payer sources and programs.
• Knowledge of regulatory guidelines for discharge planning.

Responsibilities

• Assess clinical, psychosocial, financial, and cultural needs to guide discharge planning.
• Coordinate post-hospital care and services prior to discharge.
• Lead multidisciplinary teams to execute safe, efficient discharges.
• Communicate discharge plans clearly with patients, families, and care teams.
• Document care plans and monitor milestones through discharge.

Education
