Job summary

UPMC Altoona is seeking a part-time Discharge Planning Associate to join the Clinical Care Coordination and Discharge Planning team. The role works 20 hours per week, Monday through Friday daylight hours, with weekend and holiday rotation.

You will collaborate with RNs, social workers, and physicians to identify factors shaping discharge plans and coordinate post‑hospital arrangements. The starting salary ranges are provided by level within the Discharge Planning ladder.

Responsibilities

• Identify clinical, psychosocial, historical, financial, cultural, and spiritual factors that shape the discharge plan.
• Consider patient/family/caregiver health literacy and engagement when developing the plan of care.
• Evaluate social determinants of health and their impact on transition risks.
• Complete comprehensive assessments to determine self‑care capacity and post‑hospital needs.
• Continually reassess the discharge plan in response to changing clinical or social factors.
• Lead interdisciplinary teams in developing and executing safe discharge plans.
• Maintain knowledge of local resources and post‑hospital services.
• Ensure post‑hospital arrangements are made prior to discharge to avoid delays.
• Integrate patient goals, clinical assessments, risks, and resources to support transitions.
• Serve as a liaison between hospital, post‑hospital facilities, and physicians.
• Communicate with patients and care teams to build individualized discharge plans.
• Collaborate with practitioners and nurses to coordinate care.
• Incorporate recommendations and test results into the discharge plan.
• Monitor progress toward discharge milestones and adjust plans as needed.
• Advocate for patient safety and access to needed services.
• Align practice with organizational values and regulatory standards.
• Maintain clinical knowledge and ensure compliance with regulatory requirements.
• Balance cost of care with safety, quality, and satisfaction; document planning activities.
