Job summary

Whole Life is seeking compassionate Direct Support Professionals to provide person-centered care to individuals with developmental and/or physical disabilities in Connecticut. The role focuses on promoting independence, assisting with daily living activities, and supporting community integration while maintaining safety and dignity.

Responsibilities include ADLs, medication support, documentation, and collaboration with managers, families, and care teams to implement individualized care plans.

Qualifications

• High School diploma/ GED equivalent.
• Valid driver's license and social security card, birth certificate, or passport.
• Pass background check and health screenings; post-hire physical and TB test within 30 days.

Responsibilities

• Assist with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and meal prep.
• Administer medications, monitor vital signs, and support medical appointments.
