Job summary

Whole Life is seeking compassionate Direct Support Professionals to provide person-centered care for adults with developmental and/or physical disabilities in Connecticut. You will assist with daily living activities, monitor health needs, and support client independence in a nurturing, respectful environment.

The role emphasizes teamwork with managers, families, and care plans, ensuring safety and dignity while promoting community integration and meaningful daily experiences.

Qualifications

• Proof of High School diploma or equivalent.
• Proof of valid driver's license and identity documents.
• Background checks and health screenings required; Fit for Duty and TB tests within 30 days.
• Ability to stand, move, lift up to 100 lbs and perform ADLs throughout shift.

Responsibilities

• Assist with Activities of Daily Living (ADLs): bathing, dressing, grooming, toileting, and meal preparation
• Administer medications, monitor vital signs, and support medical appointments
• Ensure a clean, safe living environment through light housekeeping and laundry
• Engage clients in social activities, community outings, and choice-making to promote independence
• Use mobility equipment (e.g., Hoyer lifts) with proper techniques
• Document daily activities, observations, and incidents accurately.
• Maintain confidentiality per HIPAA regulations.
• Implement individualized care plans
