Job summary

Alpha Health Services in Pennsylvania seeks a part-time DSP to work one-on-one with a participant in the community and at home, following the ISP, providing transportation, promoting independence, and documenting progress. You will transport to activities, assist with personal care, support social inclusion, monitor changes in health, and ensure confidentiality while collaborating with the care team.

Qualifications

• DSP experience and direct care experience required.
• Strong communication and interpersonal skills preferred.
• Ability to promote independence and document progress.

Responsibilities

• Transport participant to activities and appointments per ISP goals.
• Provide personal care with dignity and respect.
• Support community participation and social inclusion.
• Monitor health/behavior and report changes to supervisor.
• Observe and document progress and status changes.
• Implement individualized care plans tailored to the participant's needs.
• Assist with meal preparation respecting dietary needs.
• Maintain HIPAA confidentiality.
• Collaborate with care team including supervisors, healthcare professionals, and family.
• Complete PA ODP training and maintain certifications.
