Job summary

PACS is seeking a compassionate Certified Nursing Assistant to provide routine daily nursing care to residents in our post-acute care setting. You will work under supervision to support assessments, record keeping, and safety protocols.

The role requires strong communication, ability to follow care plans, maintain resident dignity, and ensure timely response to calls. CPR preferred; state CNA license required.

Qualifications

• Active CNA license required.
• CPR certification preferred.
• Ability to read and interpret care plans.

Responsibilities

• Identify residents before treatments using wristband or photo.
• Record entries accurately on charts and flow sheets.
• Report changes in resident's condition to the Nurse Supervisor/Charge Nurse.
• Assist residents with daily activities and personal care.
• Assist with meals and feeding as needed.
• Maintain confidentiality of resident information and HIPAA compliance.
• Respond promptly to resident calls and emergencies.
• Follow facility policies and safety procedures.
• Prepare the resident’s room and equipment for procedures and transfers.
