Qualifications

Responsibilities

• Maintain and update reports for the Patient Care Health Home team.
• Complete audits for invasive procedures and in-house labs.
• Manage and track all referrals, ensuring required forms are completed.
• Respond to incoming calls and emails regarding referrals.

Skills

Bilingual (English/Spanish) speaking skills

Basic proficiency with EMR, Word and Excel

Ability to prioritize work

Ability to effectively interact with public and staff

Education

High School Diploma or GED

Associates Degree or Certificate from an accredited technical school
