Job summary

Canyon Vista Recovery Center in Mesa, AZ is seeking a Part-Time Case Manager to support the treatment team. You will coordinate care across providers, implement service plans, and begin discharge and aftercare planning at admission.

Onsite only, 30 hours weekly (Fri-Mon). Requires a High School Diploma or equivalent; a Bachelor's in Social Work or related field is preferred. CPR and de‑escalation training are required; first aid may be required.

Qualifications

• High School Diploma or equivalent; Bachelor's degree preferred.
• 1+ years previous related experience in a behavioral setting preferred.
• Previous experience in case management preferred.
• CPR certification and de-escalation training required (training available upon hire). First aid may be required.

Responsibilities

• Develops and implements activities within scope of practice.
• Maintains comprehensive clinical records and progress notes.
• Coordinates care with treatment team, providers and family.
• Begins discharge planning at admission and coordinates post-discharge needs.
• Coordinates referrals and AHCCCS applications for eligible clients.
• Ensures transition, discharge and aftercare planning before service discontinuation.
• Provides crisis intervention as needed.
• Communicates client information to the treatment team and documents goals.
