Job summary

Canyon Vista Recovery Center is seeking a Part Time Case Manager in Mesa, Arizona. You will coordinate care across multiple units, ensuring discharge and aftercare planning aligns with individualized service plans, while maintaining communication with treatment teams and referral sources.

This on-site role involves collaborating with the clinical team, supporting crisis interventions when needed, and documenting progress as part of routine service delivery. Schedule: Fri-Mon, 30 hours weekly.

Qualifications

• High School Diploma or equivalent required, Bachelor's degree in Social Work, Counseling or related human service field preferred.
• One or more years previous related experience in a behavioral setting preferred.
• Previous experience with the client population served by the facility preferred.
• Previous experience in case management preferred.

Responsibilities

• Develops and implements activities and programs within scope of practice per ISP.
• Collaborates with client and family to implement effective service plans and explain options.
• Maintains comprehensive clinical records including assessments and progress toward recovery.
• Coordinates care with clinical team and community resources to implement ISP.
• Ensures discharge planning begins at admission and collects collateral input.
