Canyon Vista Recovery Center in Mesa, Arizona is seeking a Part Time Case Manager to join our treatment team. This onsite role focuses on coordinating care across providers, ensuring services align with the individualized service plan (ISP), and managing discharge and aftercare planning for clients.

The position emphasizes collaboration with families and referral sources, thorough documentation, and timely contingencies for financial entitlements.

Qualifications

• High School diploma or equivalent required; Bachelor's in Social Work or related field preferred.
• One+ year of behavioral health experience preferred.
• Experience with case management preferred.

Responsibilities

• Develops and implements activities within scope of practice per ISP.
• Collaborates with client and family to implement service plan.
• Maintains comprehensive clinical records and progress notes.
• Coordinates care with providers to implement ISP.
• Begins discharge planning at admission and secures collateral input.
• Coordinates referrals and ensures services are updated monthly.
• Assesses financial entitlements (AHCCCS, SSI/SSDI) for caseload clients.
• Develops transition, discharge, and aftercare plans before discontinuation.
• Maintains timely communications with referral sources and treatment providers.
• Uses crisis intervention techniques when needed.

Part Time Case Manager | Canyon Vista Recovery Center | Mesa, Arizona

About the Job:

PURPOSE STATEMENT:

The Case Manager supports the department by providing case management to the different units within the facility. As a member of the treatment team, is responsible for coordinating care across multiple providers to ensure clients receive support identified within the individualize treatment plan. Responsible for the overall discharge and aftercare planning needs and placement of the client. Maintains ongoing communication with the treatment team. Maintains routine contact with referral sources and providers while maintaining resource file. Participates in quality care monitoring for the department.

Schedule: Fri-Mon 30 hours weekly

Roles and Responsibilities:

• Develops and implements appropriate activities and/or programs within their scope of practice. Implements services to clients in accordance with their individualized service plan (ISP).
• Collaborates with the client and client’s family or significant others to implement an effective service plan, explaining the available options to the team, including the advantages and disadvantages of each option.
• Maintains the client’s comprehensive clinical record, including documentation of activities performed as part of the service delivery process (e.g., assessments, provision of services, coordination of care, discharge planning) and client’s progress toward their recovery.
• Works collaboratively with the clinical team to engage, educate, communicate, and coordinate care with client, their family, behavioral and general medical health care providers, community resources and others to ensure that all services prescribed in the individualized service plan (ISP) are implemented.
• Assures that discharge planning begins at admission. Obtains collateral input from providers and supports.
• Ensures all appropriate referrals for identified services on the service plan are made and coordinated with service providers via contracted network providers or community resources; Ensures all covered services identified on the service plan are evaluated and updated monthly.
• Coordinates referral or completes the screening and assessment on all clients on caseload for financial entitlements (AHCCCS, SSI/SSD etc.); completes AHCCCS applications on all clients on caseload meeting criteria.
• Ensures the development and implementation of transition, discharge, and aftercare plans prior to discontinuation of behavioral health services.
• Ensures all contact with referral source and/or other treatment providers is done within designated time frame and documented accordingly.
• Employs appropriate crisis intervention techniques as needed.
• Communicates appropriate client information to the treatment team; participates in team discussion and evaluation of client goals, objectives and motivations.

Education/Experience/Skill Requirements:

• High School Diploma or equivalent required, Bachelor’s degree in Social Work, Counseling or related human service field preferred.
• One or more years previous related experience in a behavioral setting preferred.
• Previous experience with the client population served by the facility preferred.
• Previous experience in case management preferred.

Licenses/Designations/Certifications:

• CPR certification and de-escalation training required (training available upon hire and offered by facility).
• First aid may be required based on state or facility requirements.

Work Location:

This position is onsite at the facility and is not a remote position.

Supervisory Requirements:

This position is an Individual Contributor.
