Job summary

Canyon Vista Recovery Center is seeking a Case Manager to coordinate care across multiple providers and ensure discharge and aftercare planning aligned with each client's ISP.

You will maintain client records, collaborate with the treatment team, and support referrals and entitlement processes while working on-site in Mesa, AZ. This role emphasizes coordinated care and client-centered planning.

Qualifications

• High School Diploma or equivalent; Bachelor's degree preferred.
• 1+ year experience in a behavioral health setting preferred.
• Experience with the client population served preferred.
• Previous case management experience preferred.

Responsibilities

• Coordinate care across providers to implement the individualized service plan (ISP).
• Ensure discharge planning begins at admission and is updated.
• Maintain comprehensive clinical records and progress notes.
• Coordinate referrals and financial entitlements (AHCCCS, SSI/SSD) as needed.
