Job summary

Worcester State University is seeking a part-time CARE Team Coordinator to provide individualized case management, coordinate interventions, and connect students with campus and community resources. The role includes follow-up, status updates on active cases, and liaison work with faculty, staff, parents, and other stakeholders.

Qualifications

• Minimum of 2 years of professional experience in case management, crisis intervention, or student advocacy, preferably in a college/university setting.
• Strong interpersonal skills and excellent verbal and written communication skills with the ability to communicate effectively with a variety of audiences (students, parents, faculty, staff, administration and community).

Responsibilities

• Manage a caseload of students using a student-centered approach to support success and well-being.
• Evaluate Care Team referrals and recommend relevant interventions and support strategies.
• Initiates outreach to students to assess needs, coordinate support, and connect them to appropriate resources.
• Maintains accurate, timely, and confidential documentation in designated case management and University systems (Maxient, Early Support, and Excel)
• Provide education and training to the campus community to promote awareness of the CARE Team and strengthen coordinated support for students.
• In collaboration with the Associate Director of Community Standards, track trends using case management data to identify gaps and areas of improvement.
• Develop and implement surveys to identify process improvement measures.
• Serves as a Campus Security Authority (CSA).
• Other duties as assigned.
