Qualifications

• Bachelor’s degree required; masters preferred in nursing, social work, counseling.
• Crisis management and case management experience
• Excellent interpersonal skills including the ability to communicate effectively and develop and maintain stakeholder relationships.
• Knowledge of mental health and substance use disorders, suicide prevention, young adult development, and learning theories.
• Ability to manage multiple priorities and complete tasks in a timely manner.

Responsibilities

• Establishes relationships with on-campus and community partners to gain awareness of intake, discharge processes for hospitals and IOPs, and knowledge of services provided by on-campus partners.
• Provides appropriate referrals and case management based on communication with professionals to ensure student safety.
• Acts as advocate/liaison between client, CCSD therapist, referrals, and health insurance provider.
• Consults with students, professionals, and faculty related to care navigation.
• Maintains case records in a confidential manner adhering to FERPA regulations.
• Maintains up-to-date knowledge of Delaware laws and applicable acts (FERPA, Title IX, Clery).
• Provides outreach to others on campus as a representative of the center.
• Maintains ongoing review of referral outcomes for students and providers.
• Collects and analyzes data related to case management and referrals.
• Develops and engages with local/regional mental health providers regarding services and access.
