This position requires experience in home care, strong organizational skills, a valid driver's license, and the ability to work independently. You will interact with families, caregivers, and the nursing team, with a predictable three-day schedule and steady hours.

Qualifications

• Experience in home care, home health, or Medicaid-funded services.
• Comfort with scheduling systems and LTSS familiarity.
• Valid driver’s license, reliable vehicle, and willingness to drive across five counties served.
• Able to work independently and own the operation on scheduled days.
• Clear, warm communication with caregivers, families, and planners.

Responsibilities

• Fill open shifts and match caregivers to clients; arrange replacements when needed.
• Own the master schedule for all clients and hours.
• Recruit and onboard caregivers; complete files before start.
• Visit client homes to introduce caregivers and verify care delivery.
• Correct missed clock-ins/outs in LTSS on the day they are flagged.
• Maintain caregiver training, certifications, and portal access; chase expirations.
• Schedule client assessments and coordinate with nursing team.
• Check in with support planners weekly.
