Job summary

RHA Health Services, LLC is seeking a CAP Nurse Case Manager (Part-Time RN) to support children with intellectual and developmental disabilities in Watauga, Avery, and Burke counties. You will coordinate care and collaborate with RNs and a multidisciplinary team to keep clients safe at home.

Ideal candidates have a social work or human services background with case management experience, strong communication, and a passion for person-centered care.

Qualifications

• Bachelor’s degree in social work or in a human services field with related experience
• Background checks: criminal, OIG exclusions, child abuse registries
• Directly related experience includes case management, assessment, referrals, counseling, intervention, and treatment planning

• Provide basic case management to medically fragile children in their primary residence
