Job summary

The Center for Human Development (CHD) in Western Massachusetts is seeking a part-time TBI Outreach Caseworker who is bilingual (English/Spanish) to support participants with traumatic brain injuries living in Westfield, West Springfield and surrounding communities.

You will provide direct services, assist with Individual Service Plan development, help clients obtain entitlements, manage ongoing health needs, document progress, and travel for home visits with mileage reimbursement.

Qualifications

• Bachelor’s Degree or equivalent experience.
• Valid driver’s license and ability to provide transportation as needed.
• 21 years of age or older.
• Bilingual with proficiency in English and Spanish.

Responsibilities

• Support participants to achieve goals, manage health needs, and build independent living skills.
• Help obtain and maintain entitlements, manage funds, and create social/recreational opportunities.
• Provide and maintain written documentation and assist ISP development and progress updates.
