The Center for Human Development (CHD) in Western Massachusetts is seeking a part-time TBI Outreach Caseworker who is bilingual (English/Spanish) to support participants with traumatic brain injuries living in Westfield, West Springfield and surrounding communities.
You will provide direct services, assist with Individual Service Plan development, help clients obtain entitlements, manage ongoing health needs, document progress, and travel for home visits with mileage reimbursement.

The Center for Human Development (CHD) is seekinga part time TBI Outreach Caseworker (Bilingual) to join our team! The TBI Outreach Caseworker is responsible for providing direct services and casework support to program participants with Traumatic Brain Injuries (TBI) who live independently in the greater Westfield/West Springfield and Springfield community.
The ideal TBI Outreach Caseworker will requirestrong interpersonal skills, empathy for others, and a demonstration of commitment to serving those with significant needs. The ability to work well within a team model and independently will also make you successful. This role requires travel for home visits throughout Western Mass, including Hampden, Franklin and Hampshire counties with mileage reimbursement.
