Job summary

The Center for Human Development (CHD) is seeking a part-time TBI Outreach Caseworker (Bilingual) to join our team in the greater Westfield/West Springfield and Springfield area. The role involves direct services and casework support for participants with Traumatic Brain Injuries who live independently.

Responsibilities include supporting participants to achieve goals, assisting with health maintenance, entitlements, and developing ISP documentation, travel in Western MA with mileage

Qualifications

• Bachelor’s Degree or equivalent experience.
• Valid driver’s license and ability to provide transportation on an as needed basis.
• 21 years of age or older.
• Bilingual with proficiency in both English and Spanish required.

Responsibilities

• Support program participants to achieve their individual goals and objectives, manage ongoing health maintenance needs, and provide supports/skill building for independent living.
• Help people obtain and maintain entitlements, manage funds, and develop social and recreational experiences.
• Provide and maintain written documentation and assist Case Manager with ISP development, assessments, and progress updates.
