Job summary

CHR Community Health Resources, Inc. seeks an Outreach Therapist for its Intimate Partner Violence program in Hartford, CT. This part-time role covers 20 hours weekly, with supervision and ongoing training opportunities in evidenced-based models.

You’ll work with families impacted by violence, delivering assessments, risk evaluations and tailored treatment plans across office, home, or community settings.

Qualifications

• Master’s Degree or higher in related field.
• Experience with trauma, mental illness or SUD preferred.
• Licensure/certification: LMSW, LPC-A, LMFT-A required; LCSW/LMFT/LPC or licensed psychologist preferred; LADC may be substituted.

Responsibilities

• Provides assessment and treatment interventions consistent with the program model.
• Conducts clinical and risk assessments.
• Creates individualized, person-centered treatment plans.
• Maintains client and staff records per policy.
• Provides crisis assessment and intervention as needed.
• Possibly administers care in office, home, or community settings.
