Grand Street Settlement in New York, NY is seeking a Part-time Bilingual Spanish Case Worker to support the BEST/NORC program for homebound seniors. The role focuses on intake, case planning, and coordination of services to meet health and psychosocial needs, with emphasis on preventing social isolation.

Responsibilities include intake assessments, goals and timelines, assistance with Medicaid and food assistance, and data entry into program databases.

Formación

• BA in Social Work, Psychology, or related field preferred or Associate Degree in Human Services.
• Excellent communication, analytical and problem-solving skills.
• Bilingual English/Spanish.
• Experience delivering social services to seniors.
• Strong computer skills: Excel, Word, and data entry.

Responsabilidades

• Provides case assistance and case management to participants and documents interventions in client files.
• Identify and provide NORC assessment to homebound seniors at risk of social isolation.
• Conduct initial interview and intake assessment; formulate goals and timelines with clients.
• Assist participants in obtaining entitlements including Medicaid, food stamps, SCRIE, home care referrals.
• Complete enrollment of NDA clients, follow-up notes and outcomes; enter data into funder databases.
• Provide family development coaching sessions to participants and translation services when needed.
• Assist in GMHI screening to support increasing Baruch community’s mental health awareness.
• Coordinate program events and conduct home visits as required.
• Connect seniors to GSS Senior Center/NORC/Essex Crossing activities.
• Support in food distributions/food pantry and related tasks.
