Job summary

The Columbus Organization in Indianapolis, IN seeks a Part-time Care Coordinator to manage service coordination for individuals with I/DD, ensuring access to waivers, Medicaid State Plan, and community supports within a person-centered framework.

You will advocate for individuals, ensure compliance with Indiana DDRS, BQIS, and organizational standards, and collaborate with families, guardians, and providers. Bilingual English/Spanish required, and travel within Marion County is expected.

Qualifications

• Bachelor's degree in a human services-related field or equivalent experience.
• One year of full-time direct experience with individuals with intellectual/developmental disabilities (I/DD) or RN with related experience.
• Four years of relevant human services experience in lieu of a bachelor’s degree with at least one year of I/DD experience.
• Demonstrated initiative, accountability and ability to work independently.
• Strong interpersonal and relationship-building skills.
• Bilingual in English and Spanish.

Responsibilities

• Serve as primary liaison with waiver participants, families, guardians, providers, and state agencies.
• Ensure confidentiality and HIPAA compliance in all records and communications.
• Develop person-centered plans (PCISPs) aligned with individual goals.
• Conduct annual planning activities, budget development, and LOC determinations.
• Lead IST meetings at least semi-annually with participant involvement.
• Perform in-person visits, including annual in-home visits and provider settings when applicable.
