Job summary

Constellation Health Services is seeking a Hospice Bereavement Coordinator to assist with virtual support groups and patient/family bereavement services. The role involves planning, implementing, and maintaining a bereavement program for up to one year after a loss, with an estimated 15–20 hours per week on a flexible schedule.

The coordinator will assess needs, coordinate follow-up, maintain records, and provide information and referrals to the community.

Qualifications

  • Bachelor’s degree or equivalent theological degree or Counseling degree from college or university.
  • Experience with families/caregivers and patients through death/dying issues – OR – master’s degree in social work or counseling with qualifications of experienced clinical grief counseling.
  • Ability to work as a team member.
  • Knowledge and commitment to hospice philosophy.
  • Demonstrates good verbal and written communication, and organization skills.
  • Understanding of grief response and experience with counseling individuals and/or in groups regarding loss

  • Develops, coordinates and evaluates bereavement follow-up programs.

  • Maintains appropriate records and files of bereaved enrolled in the program and documents follow-up.
  • Provides bereavement information and referral services to callers from the community.
  • Expands and maintains community resource list as needed.
  • Plans, publicizes, facilitates and supervises bereavement support groups and programs.
  • Plans and oversees events such as Hospice Memorial Services and Bereavement Care Seminars.

Constellation Health is seeking a Hospice Bereavement Coordinator to assist with our Virtual Support Groups Constellation Health is looking for a compassionate Bereavement Coordinator to work with hospice patients and their families in their homes. The Bereavement Coordinator is responsible to plan, implement, and maintain a bereavement program to meet the needs of families/caregivers from Constellation Hospice for up to one (1) year following the death of the individual hospice patient.
