Job summary

Covenant Living Communities and Services is seeking a Bereavement Coordinator to plan, implement, and maintain a bereavement program for families and caregivers up to one year after a patient’s death in CovenantCare Hospice.

The role emphasizes grief counseling services, coordination with the community, and participation in hospice activities. A Master’s in Social Work with clinical grief counseling qualifications is required.

Qualifications

  • Master's degree in Social Work required with qualifications in clinical grief counseling.
  • Experience with families, caregivers and patients on death/dying issues desired.
  • Strong communication, teamwork, and organizational skills essential.

Responsibilities

  • Coordinate grief counseling services (one-to-one and groups) for families/caregivers and follow up.
  • Provide bereavement information and referrals to community resources.
  • Participate in in-service education, meetings, and relevant committees.
  • Support hospice personnel coping with grief through counseling and referrals to resources.
  • Assist in evaluating services to meet personnel support needs (e.g., retreats, support groups).
  • Work independently while collaborating with the interdisciplinary team.

The Bereavement Coordinator is responsible to plan, implement, and maintain a bereavement program to meet the needs of families/caregivers from CovenantCare Hospice for up to one (1) year following the death of the individual hospice patient.

Coordinates the assessment and delivery of grief counseling needs and services (one-to-one, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers.

Provides bereavement information and referral services to callers from the community. Expands and maintains community resource list as needed.

Participates in hospice activities such as in-service education, hospice personnel meetings, and relevant committees.

Provides bereavement support to hospice personnel coping with work related grief through one-to-one counseling, supervision of interns providing one-to-one counseling, and referral to community resources.

Assists as needed in evaluating and planning services to meet hospice personnel support needs (i.e., yearly retreats, weekly support groups).

Functions independently, but seeks and accepts guidance from other members of the interdisciplinary group, or from member of the community.

Actively participates in quality assessment performance improvement teams and activities.

Education And Work Experience

Master's degree in Social Work with qualifications of experienced clinical grief counseling.

Experience

  • Experience with families/caregivers and patients through death/dying issues
  • Experience with clinical grief counseling.

Knowledge, Skills And Ability

  • Understanding of grief response and experience with counseling individuals and/or in groups regarding loss.
  • Ability to work as a team member.
  • Knowledge and commitment to hospice philosophy.
  • Demonstrates good verbal and written communication, and organization skills.

Physical Requirements

This position requires mobility to move around from one area to another. The majority of time is spent sitting, walking, bending, stooping and driving. Employee must have the ability to lift loads not normally exceeding medium weight (35 pounds). Manual dexterity, keen vision and hearing accuracy are necessary in driving a vehicle.

Compensation Pay Range

$23.62 - $32.16 per hour

Reasonable Pay Estimate

A reasonable estimate of the pay range for this position is $23.62 - $32.16 per hour. There are numerous factors taken into consideration in determining the actual offered rate of pay, including but not limited to: job-related qualifications, experience, skills, education, geographic location, and consideration of internal and external equity.
