Job summary

EMPACT-SPC/La Frontera Arizona in Maricopa, AZ seeks a part-time Rehabilitation and Discharge Planner to coordinate rehabilitation services for adults with serious mental illness and assist in discharge planning.

The role combines rehabilitation and discharge planning duties, requiring collaboration with RSA counselors, providers, and the treatment team. A bachelor’s in behavioral health is required, master’s preferred, and 2+ years of case management experience. Sign-on bonus up to $2000.

Qualifications

• Bachelor’s degree in Psychology, Social Work, Family Studies, or related behavioral health field required.
• Master's degree preferred.
• Minimum of 2 years of case management or inpatient behavioral health experience required.
• Must have, or be able to obtain a Level 1 fingerprint clearance card.

Responsibilities

• Coordinate and monitor rehabilitation-related services for consumers with serious mental illness.
• Provide case management and coordination for Seriously Mentally Ill adults.
• Collaborate with RSA Vocational Rehabilitation Counselors, providers and others to implement the IGA at direct service level.
• Educate team members on rehabilitation and vocational issues and provide ongoing training.
• Complete required documentation and data collection reports as requested.
