Job summary

EMPACT-SPC is seeking a Part-Time SMI Rehabilitation and Discharge Planner to coordinate rehabilitation services for adults with serious mental illness. You will perform case management and help develop discharge plans within the inpatient and community settings.

Responsibilities include collaborating with RSA VR counselors, documenting plans, and tracking outcomes. The role requires at least 2 years in behavioral health case management and strong organizational skills; travel to multiple sites

Qualifications

• Bachelor’s degree in Psychology, Social Work, Family Studies, or other behavioral health related field required.
• Master's degree preferred.
• Minimum of 2 years of case management or inpatient experience in behavioral health required.

Responsibilities

• Coordinate and monitor rehabilitation-related services for adults with serious mental illness.
• Provide case management and discharge planning for Seriously Mentally Ill consumers.
• Collaborate with RSA Vocational Rehabilitation Counselors and providers to implement IGA.
• Document treatment plans, readiness assessments and progress reports.
• Complete tracking and data collection reports for Employment and Rehabilitation Services.
