Job summary

Codac Health Recovery & Wellness in Tucson, AZ is seeking a Discharge Planner to coordinate hospital stays and discharge planning for adult members, in collaboration with the adult recovery teams and Utilization Management.

The role requires a healthcare degree or equivalent experience, plus strong knowledge of community resources and behavioral health. A valid Arizona driver’s license is required; travel is more than 90 percent.

Qualifications

• Master's or Bachelor's degree in a healthcare field and no related experience OR;
• Master's or Bachelor's degree in a non-healthcare field and 1 year related experience OR;
• Associates degree in a healthcare field and 1 year related experience OR;
• Associates degree in a non-healthcare field and 2 years related experience OR;
• Certified Peer Support Specialist and 2 years related experience OR;
• High School diploma or GED with 4 years healthcare experience.
• Arizona Licensed Registered Nurse OR;
• Arizona Licensed Physician Assistant who is not working as a medical practitioner.

Responsibilities

• Initiate discharge plans for new and recent admissions.
• Prepare documentation including intake and release forms.
• Coordinate with adult recovery team and site staff for post-discharge needs.
• Communicate with hospital discharge planners and other staff on discharge progress.
• Ensure discharge summaries are sent to medical records and coordinate referrals.
