Jawonio Inc. seeks a part-time Behavioral Health Care Manager to provide person-centered care coordination for adults with severe mental illness in the New York area. You will assess needs, connect clients to medical, behavioral health, housing, employment and benefits resources, and advocate for client rights.

The role emphasizes individualized service plans, ongoing record-keeping, and collaboration with a multidisciplinary team to improve overall functioning and prevent hospitalization.

Qualifications

• Master’s degree in social work, psychology, nursing, rehabilitation, counseling or related field with 1 year of experience, or
• Bachelor’s degree in same fields with 2 years of experience, or
• Bachelor’s degree or higher in any field with 3 years of experience serving SMI/SED populations.

Responsabilités

• Identify and assess participants for care management including medical, behavioral health, substance use recovery and social services.
• Engage participants in community, in‑home, and court settings to improve quality of life.
• Serve as a referral source connecting individuals to community resources and educate patients on diagnoses.
