Job summary

The Jewish Board is seeking Care Coordinators to connect adults and children with chronic behavioral health and medical conditions to essential services. Working in the field and in teams, you will assess needs, develop individualized care plans, coordinate care, arrange appointments, and educate members to improve health outcomes.

This role requires collaboration with families and providers, travel within New York City, and strong communication.

Qualifications

• Bachelor's degree in social work or related field or NYS licensure with degree is required.
• Two years of direct services or case management experience for chronically ill or disabled individuals is preferred.
• Experience in interdisciplinary teams is advantageous.

Responsibilities

• Integrate medical, behavioral health, and social services for members.
• Assess medical and behavioral health needs and develop ICPs with stakeholders.
• Provide required care management services and coordinate appointments.
• Document activities and maintain required records in HIT systems.
• Coordinate referrals and ensure access to prescribed services.
