Job summary

The Jewish Board seeks a Care Coordinator to connect adults and families with chronic behavioral health and medical conditions to needed services. You will assess needs, develop person-centered care plans, and coordinate referrals in a field-based role with team support.

Ideal candidates have a relevant degree or RN/teacher credentials and strong communication skills. The position requires flexibility for home visits, community settings, and some travel within New York City.

Qualifications

• Bachelor's degree in social work or related field, or NYS teaching certification, or RN with relevant experience.
• 5 years' experience with applicable population if using alternative degree.
• Experience in care coordination in medical/behavioral health.
• Fluency in a second language (Spanish, Russian, or Creole) preferred.

Responsibilities

• Coordinate integration of medical, behavioral health, and social services.
• Periodically assess medical and behavioral health needs and treatment compliance.
• Develop ICP with the member, family, and providers.
• Provide care management services and track referrals.
• Ensure access and engagement with ICP services.
• Respond to member crises during and outside business hours.
• Document all activities and maintain records.
