Job summary

Transitional Services for New York, Inc. is seeking a Care Coordinator I for its BHCC program in Jamaica, NY. The role provides health home services addressing physical, mental, and health needs to support independence and reduce ER visits.

The ideal candidate will have a health/human services background, strong communication skills, and the ability to manage care planning, coordination, and follow-up across multiple providers.

Qualifications

• Bachelor's in Health & Human Services with 1 year direct care experience, OR
• Associate's in Health & Human Services with 2 years direct care experience, OR
• HS diploma/GED with 4 years direct care experience; equity stated.

Responsibilities

• Provide outreach and engage BHCC clients.
• Develop recipient-centered, strength-based care plans.
• Assist clients in following and managing their plan of care.
• Coordinate services across systems to meet needs.
• Monitor and evaluate client needs and progress.
• Arrange services and facilitate communication with providers.
• Promote wellness resources and linkage to community supports.
• Participate in discharge planning and follow-up to ensure continuity.
