Qualifications

• Board-certified physician with experience in primary care and long-term care.
• Strong leadership to mentor NPs and PAs in delivering care.
• Comfort working in SNFs with part-time schedule.

Responsibilities

• Act as the primary care provider for residents in SNFs, overseeing medical management.
• Conduct patient rounds, collaborating with NPs and PAs to assess care needs.
• Diagnose and treat acute illnesses while managing chronic conditions.
• Discuss treatment plans, prognosis, and advance care planning with residents and families.
• Maintain accurate medical records and ensure compliance with healthcare regulations.
