Qualifications

• Phone support: answer agency calls and relay messages clearly.
• Data entry: record information and referrals into SAMS database.
• Assessment work: complete NAT-E assessments for seniors receiving meals.
• Information sharing: provide updates to the Director and supervisors.
• Documentation tasks: enter referrals, assessments and care planning details.
• Clerical duties: typing, copying, faxing and basic office tasks.

Responsibilities

• Provide updates to the Director and communicate observations to supervisors.
• Enter referrals, assessments, and care planning details into databases.
