Job summary

Wayne County Area Agency on Aging in Honesdale, PA is seeking a qualified Aging Care Manager 1 to assist senior citizens with accessing resources to promote safety, independence, and well-being.

You will begin with structured training to develop professional aging care management skills, work with a dedicated team, and contribute to coordinated services for older adults, their families, and partners.

Qualifications

• Three years as an Aging Case Aide 2 or equivalent experience in aging services.
• Bachelor's degree with twelve (12) credits in social sciences, behavioral sciences, human services, or closely related field.
• Equivalent combination of experience and training accepted.

Responsibilities

• Case Management: Manage a caseload guiding older adults and families through needed services.
• Service Coordination: Work with physicians, community programs, financial institutions and partners to support consumer needs.
• Specialized Support: Include Protective Services, Care Transitions, and Person-Centered Counseling.
