Depending on your role and your hours, we offer:
Assist Care Managers with administrative and consumer related functions to ensure that all data entry and casework processing is accurate and timely. The Case Aide handles the opening and closing of cases and entering consumer information in database.
Supports Intake processing by preparing consumer charts, creating care plans, and entering financial information into statewide consumer database for newly opened consumers.
Provides administrative support and data entry for ongoing cases including, but not limited to, program transfers and closings.
Completes documentation in consumer database and abides by regulations as set forth by agency and the Executive Office of Aging & Independence (AGE).
Attend mandatory meetings and trainings including Team Meeting, General Staff Meeting and Home Care Staff Meeting.
Other duties as assigned by Home Care Management Staff.
Associates Degree and 1 year relevant work preferred. High School diploma with significant relevant work experience may be substituted for degree. Work in a human service environment a plus. Strong computer skills including proficiency in Microsoft Office applications are required. Must have outstanding time management skills and be able to handle multiple tasks at a time. Must be responsible and have compassion for older people. Excellent interpersonal and communication skills a must in this team-oriented position.
At AgeSpan, we carefully consider a wide range of factors when determining rate of pay. Actual rate of pay depends on factors including but not limited to internal equity, experience, and qualifications. The pay range for this position is $21.00 - $22.00/hour
