Washington State Department of Social and Health Services in Port Angeles is seeking a Social Service Specialist 3 to conduct comprehensive in-home assessments and develop long-term care plans for adults, including those with disabilities or elderly individuals.

The role focuses on responsive case management, collaboration with families and facilities, and ensuring services meet clients’ needs while promoting independence and safety in the community.

Qualifications

• Experience in a mental health or behavioral health setting or providing direct services to clients with mental health conditions.
• Strong written and verbal communication abilities and collaboration with clients, peers, and management.
• Knowledge of DSHS policies, rules and processes; ability to learn new programs.

Responsabilités

• Conduct face-to-face comprehensive assessments and reassessments in home or facility settings.
• Develop and manage care plans to help clients stay in or return to the community.
• Provide case management to adults at high risk of institutionalization.
• Make referrals to community resources and consult with supervisor on cases.
• Prepare and file care documents; draft letters and responses for reports.
• Coordinate with nurses on Skin Observation Protocols and other medical indicators.
• Advocate for clients and coordinate with families and facilities.
• Attend case staffing meetings and interagency coordination meetings.
• Perform other duties as assigned.
