St. Mary’s Healthcare System for Children is seeking a Patient Service Coordinator for the Centralized Para Placement Unit in Uniondale, NY.

The role ensures initial placement of paraprofessional services and reassignments, coordinating with vendor teams to meet patient needs. Responsibilities include scheduling, vendor communications, and maintaining profiles in Cerner HomeWorks, with a focus on timely coverage and accurate service levels.

Qualificações

• Associates Degree preferably in Computer Science or the equivalent and/or related experience preferred.
• Strong knowledge of hardware, software, and telecommunications operations.
• Excellent consultative and communication skills, analytical ability, judgment, and the ability to work efficiently with management and staff.
• Mature individual, able to handle confidential aspects of the position.

Responsabilidades

• Communicates with paraprofessional vendors to cover cases promptly and informs vendors of patient needs per the Plan of Care.
• Collaborates with vendors and Home Care Team to set service levels, times, and days for cases.
• Ensures vendor profiles and signatures are updated and maintained per policies.
• Initiates paraprofessional scheduling for initial certification periods in Cerner HomeWorks.
• Monitors changes in paraprofessional services and maintains a change log for billing purposes.
