Qualifications

• Master’s degree in Social Work required.
• Must hold PA licensure as LSW or LCSW.
• Basic Life Support certification within 30 days of hire.

Responsabilités

• Coordinates management and operations of hospital aspects of the Palliative Care program.
• Provides care coordination, care plan development and assessments.
• Assists with symptom management and pain control to improve clinical outcomes.
• Oversees data management and conducts program evaluations.
• Educates patients, families, and staff; develops educational materials and trainings.
• Schedules and supports educational development within the program.
• Collaborates with care management, chaplaincy, rehab, pharmacy, and dieticians.
