Job summary

SHC WVUHS Home Care seeks a licensed social worker to collaborate with physicians and care teams in designing and documenting explicit hospice plans of care. The role involves implementing and evaluating plans, collecting data, and optimizing quality and utilization metrics in Home Care settings.

The position requires a Master’s in Social Work and current state licensure (OH/PA/MD/WV) with BLS and driver's license.

Qualifications

• Master’s Degree in Social Work or related field.
• Current licensure as required by state board where services will be provided.
• LSW/LISW/LISW-S licensure by the appropriate state board (OH/PA/MD/WV).
• Valid driver's license.
• Basic Life Support (BLS) certification within 30 days of hire.

Responsibilities

• Completes comprehensive assessment to develop a safe hospice plan of care for the patient.
• Assesses for signs and symptoms of abuse/neglect and makes referrals to appropriate agencies.
• Identifies decision makers and assists with Living Will/POA forms as requested.
• Plans, implements and evaluates QI strategies.
• Provides counseling to meet psychosocial, emotional and spiritual needs of patients and families.
• Refers patients and families to community services; complete referral forms as needed.
• Documents for IDT meetings and develops an interdisciplinary care plan addressing needs.
