Position Overview

Please note: This position will focus on inpatient adult palliative care. It reports to the Case Management and Social Work Supervisor and is accountable to the Palliative Team.

Job Summary

The incumbent functions as the Palliative Care Social Worker for inpatient and outpatient palliative care at the University of Vermont Medical Center Palliative. The role involves participation in palliative care consultations as part of a multidisciplinary team, engaging in serious illness conversations, and assessing sources of suffering in psychological and social domains. Responsibilities include supporting plan of care coordination, ensuring continuity during hospital stays, and providing psychological, social, and bereavement support. The position also involves attending to family dynamics, supporting coping mechanisms, identifying resources, and mediating conflicts.

This position will support and participate in after-hour and weekend coverage to ensure 24/7 organizational coverage for regulatory and accreditation purposes.

Education

Master's in social work from an accredited school of social work. LICSW required. Palliative Care/Case Management certification by a recognized accrediting body is preferred or willingness to obtain within 3 years (examples include CCM, ACM, ANCC, or ACHP-SW).

Experience

Minimum of 1 year of experience in Palliative Care or Hospice, and at least 3 years of experience in Case Management.
