Job summary

Intermountain Health is seeking a compassionate Palliative Care RN for our St Vincent Regional Hospital in Billings, Montana. You will coordinate care, educate patients and families, and collaborate with an interdisciplinary team to manage symptoms and advance care planning.

Qualifications

• Current RN license in the practicing state; compact license transfers within 60 days.
• BSN from an accredited program; education verified.
• BLS certification for Healthcare Providers.
• RNs with <12 months experience must obtain BSN within 4 years of hire.

Responsibilities

• Coordinate care and serve as a navigator for palliative care patients and their families.
• Provide patient and family education regarding diagnosis, treatment options, and available resources.
• Assess symptoms and collaborate with providers to support symptom management plans.
• Triage patient and family calls, questions, and care concerns.
• Coordinate services across primary care, specialty care, hospice, home health, and community resources.
• Support advance care planning conversations and documentation.
• Provide ongoing follow-up and care coordination throughout the patient's healthcare journey.
