Job summary

TriHealth Hospice PalliaCare Social Worker plays a key role in supporting patients with serious illnesses and their families at home, coordinating care and linking to community resources to improve quality of life. Travel to patient homes is required, with a Monday–Friday schedule and no weekends or holidays.

The role emphasizes collaboration with nurses, physicians, and a multidisciplinary team to ensure compassionate, patient-centered care and smooth transitions across levels of care.

Qualifications

• Master's Degree in Social Work (or related field) and Ohio LSW required.
• LISW preferred.

Responsibilities

• Assess the psychosocial needs of the patient and family to develop, communicate, and implement an effective plan for psychosocial interventions.
• Collaborate with the multidisciplinary team to ensure effective, efficient, appropriate care.
• Develop and implement plan of care based on assessment including counseling or crisis intervention and link to community resources.
• Provide on-call services according to departmental policy and respond to referrals.
