Job summary
TriHealth's Hospice Care Team seeks a dedicated Registered Nurse to deliver holistic, patient-centered care to patients and families in home-care settings across the Greater Cincinnati region. You will collaborate with physicians and other health care professionals, perform assessments, develop and implement care plans, and educate patients and families while maintaining compassionate, high-quality care.
Travel within the region is required; competitive benefits and shift differentials are
Qualifications
- Requires ADN or diploma in nursing.
- RN license is required.
- New hires must obtain a BSN within 5 years.
- Up to 1 year clinical nursing experience is required.
- Knowledge of holistic, patient-centered care and ethical decision-making.
Responsibilities
- Performs initial and ongoing patient and family assessments.
- Plans care based on assessment data and standards of care; updates plans as needed.
- Maintains a safe, therapeutic environment and respects patient autonomy.
- Evaluates care plans and collaborates with the health care team to revise plans.
- Administers medications/treatments per policy and supervises other staff as needed.
- Pursues professional development and contributes to continuous improvement.
