Faith Palliative Care in Michigan is seeking a Nurse Practitioner or Physician Assistant to join our team as a Palliative Care Advanced Practice Practitioner in a full-time role. You’ll provide compassionate, individualized care to people living with serious or chronic illness, focusing on symptom management and goal-concordant planning.

Responsibilities include comprehensive consultations, ongoing assessments, collaborative care with physicians and the team, and educating families about options

Formación

• Strong clinical judgment and the ability to practice independently while collaborating with a team.
• Proficiency with electronic medical records and other clinical technologies.
• Excellent communication with patients, families, and care team.

Responsabilidades

• Conduct comprehensive palliative care consultations in a variety of settings.
• Provide ongoing care, including assessment, symptom management, and evaluation of treatment effectiveness.
• Collaborate with patients’ treating providers, palliative care physicians, and other members of the care team.
• Educate and counsel patients, families, and caregivers about symptoms, treatment options, and goals of care.
• Participate in the weekend hospital rounding rotation
