Job summary
Anew Palliative Care in Indiana is seeking a Nurse Practitioner (NP) to provide comprehensive home care services under physician supervision, delivering palliative care in patients' homes.
The NP will perform assessments, develop care plans, prescribe within licensure, coordinate with families and the interdisciplinary team, and support a patient-centered approach.
This PRN opportunity offers scheduling flexibility and a supportive team committed to compassionate service.
Qualifications
- Master's degree in nursing with NP training in palliative/hospice care.
- Current state NP licensure and CPR certification.
- 1 year NP experience in palliative or hospice care preferred; CHPN certification a plus.
Responsibilities
- Provide medical care and manage terminal illness in compliance with nursing practice acts.
- Perform history and physical examinations and document findings.
- Develop individualized care plans, prescribe meds within licensure, coordinate hospice services.
- Collaborate with interdisciplinary team to ensure symptom control and quality care.
- Review and develop care protocols; consult with physician when plans exceed standard protocols.
