Responsibilities

• Provide palliative care consultations under physician supervision.
• Complete history and physical exams and document findings.
• Develop and manage individualized plan of care; prescribe meds as needed.
• Coordinate hospice care and referrals with the care team.
• Educate patients and families on disease trajectory and care options.
• Maintain accurate clinical notes and orders and ensure safe patient environment.
