Job summary

Ohio Living seeks an Advanced Practitioner to provide patient care during home, facility, and clinic visits in accordance with standards and scope of practice. You will diagnose, treat, prescribe, and maintain comprehensive EMR documentation while collaborating with a high-performing medical team.

You must hold a current Ohio NP or PA license, possess narcotics licenses, and bring at least two years of advanced practice experience, with geriatrics preferred.

Qualifications

• Current unencumbered license for the state of Ohio to practice as a Certified Nurse Practitioner or Physician Assistant.
• Narcotics and controlled substances licenses/permits for Medicare and Ohio Medicaid.
• Two years of experience as an advanced practitioner; geriatrics experience preferred.

Responsibilities

• Care and Relationship Management: interpret symptoms, obtain histories, diagnose, develop treatment plans, and provide patient education.
• Operations Management: assist in program development and educational initiatives.
• Quality & Compliance: help update policies and ensure regulatory compliance.
• Maintain EMR documentation and collaborate with physicians and nursing staff.
