Job summary

UnitedHealth Group seeks an Advanced Practice Clinician (NP/PA) in Ohio to provide acute and chronic care at home, in assisted living facilities, and SNFs. You will work across the Hamilton and Butler County markets with a Senior Living Program and SNF Patient Connect initiatives.

The role involves one-time patient visits, documentation with CMS standards, and collaboration with case management to improve transitions and outcomes.

Qualifications

• Active licensure in the state of practice
• Driver's license and reliable transportation
• 40 hours per week
• Proficiency with electronic medical records and documentation

Responsibilities

• Perform comprehensive assessments and document findings per CMS regulations
• Support members during short-term sub-acute rehab episodes
• Ensure diagnoses are ICD-10 coded and documented correctly
• Ensure all quality elements are addressed and documented
• Conduct initial medication review
• Attend mandated trainings and complete computer/virtual trainings
• Travel between care sites as required
• Coordinate care: communicate with patient and team members, support advanced illness planning and counseling

Education

ANCC or AANP Family/Adult/Geriatric Primary Care certification

NCCPA certification
