Job summary

We Care Health Centers in Ohio seeks a Nurse Practitioner to deliver primary and preventive care for residents in skilled nursing facilities. The NP will manage acute changes in condition and coordinate with interdisciplinary teams.

Responsibilities include advanced care planning, chronic disease management, prescribing medications per Ohio law, and collaborating with primary care physicians. Strong EMR experience and geriatric care background preferred.

Qualifications

• Current, unrestricted NP license in the state of Ohio.
• Board Certification and DEA licensure required.
• Experience in long-term care and EMR proficiency.
• Ability to work within an interdisciplinary care team to coordinate patient care.

Responsibilities

• Provide primary and preventative care for long-term care patients in skilled nursing facilities.
• Manage residents with acute changes in condition.
• Perform advanced care planning and chronic care management.
• Prescribe medications, including controlled substances per OH law.
• Order and follow up on treatments and diagnostic testing.
• Communicate with nursing home staff regularly about patient condition changes.
• Collaborate with primary care physicians.
