Job summary

Elevance Health is seeking an Advanced Practice Provider - Field Based Care to deliver patient-centered care predominantly in home settings within the Ohio region. You will collaborate with physicians and other providers to design and implement comprehensive plans of care focused on adults with chronic and complex conditions.

The role requires an MS in Nursing, an NP or PA license in the applicable states, DEA registration, CPR certification, and experience with EMR systems.

Qualifications

• MS in Nursing required.
• Current unrestricted NP or PA license in applicable state(s).
• DEA registration or eligibility required.
• CPR certification required.

Responsibilities

• Provide services in home settings to patients.
• Develop and implement plans of care for adults with chronic/complex conditions.
• Gather history, perform exams, and implement treatment plans aligned with goals.
• Collaborate with interdisciplinary team for patient care and resources.
• Educate patients and families on medications, adherence, and prevention.
• Maintain medical records according to state regulations and policies.
• Engage in continuing education as required by state.
• Prescribe medications as permitted by state authority.
