Qualifications

• Current DEA license.
• Certification by ANA/AANP or NCC in family, adult-gerontology, or related area.
• CPR certification.
• Hospice/Homecare or palliative care experience preferred.
• Licensed to practice as NP per state requirements.

Responsibilities

• Provides direct patient care and evaluates outcomes under physician supervision.
• Collaborates with physician preceptor to formulate and review medical protocols.
• Obtains complete medical history and performs appropriate physical exams.
• Identifies health hazards and factors affecting the care plan.
• Maintains accurate documentation and required records.
